The following information applies to each date:
- Available for any student-athletes entering grades 7-12 - Individuals walk-ins welcome (NO APPOINTMENT NECESSARY) - Athletes must be dressed in shorts - OHSAA physical forms will be provided Note: IHSAA physical forms will be provided for the physicals at our Batesville location. - These forms must be properly filled out and signed by the parent and/or guardian, prior to the examination. - Cost will be $20 per athlete (cash or check only) - $10 per athlete will go back to the schools’ athletic department to purchase sports medicine supplies, in the form of a gift card voucher. |
